D.Pharm & B.Pharm Registration Transfer

For transfer of D.Pharm / B.Pharm Pharmacist Registration, applicants are required to submit the following documents:
1. Existing Pharmacist Registration Certificate
2. D.Pharm / B.Pharm Degree or Diploma Certificate
3. All Years/Semesters Mark Sheets
4. Existing Registration Number/Details
5. Government-issued Identity Proof
6. Recent Passport-Size Photograph
7. Signature
8. Transfer/NOC Certificate from the Existing Pharmacy Council, if applicable
9. Address Proof, if required
10. Transfer Application Form
11. Prescribed Transfer Fee Receipt
12. Any additional document required by the concerned Pharmacy Council
