D. Pharmacy Registration Renewal – Required Documents

Applicants are required to submit the following documents for renewal of D. Pharmacy registration:

1.Original/Existing Pharmacy Registration Certificate
2.Pharmacy or B. Pharmacy Passed Mark sheets and Certificates
3.Recent Registration Renewal Application
4.Recent Passport-Size Photograph
5.Valid Government-issued Identity Proof (Aadhaar Card/PAN Card/Passport, as applicable)
6.Permanent Resident Address must write
7. Communication Address also must write
8.Phone / Mobile No. write & E-mail Id also write

After receiving your documents, the concerned authority will verify your credentials. Upon successful verification, you will be informed to pay the prescribed ₹850 registration fee. After completion of the process, the Registration Certificate will be issued, and your name may be listed under “Sikkim Registered Pharmacists List” on the Sikkim Pharmacy Council website: www.sikkimpharmacycouncil.com
