    D.Pharm & B.Pharm Pharmacist Registration – Required Documents

    Applicants are required to submit the following documents for pharmacist registration:

    D.Pharm Registration – Required Documents

    1. D.Pharm Final/Pass Certificate
    2. D.Pharm All Years/Parts Mark Sheets
    3. 10th Class Certificate/Mark Sheet
    4. 12th Class Certificate/Mark Sheet
    5. Government-issued Identity Proof
    6. Recent Passport-Size Photograph
    7. Signature
    8. Internship/Training Certificate, if applicable
    9. Transfer/Leaving Certificate, if applicable
    10. Migration Certificate, if applicable
    11. Registration Application Form
    12. Prescribed Registration Fee Receipt

    B.Pharm Registration – Required Documents

    1. B.Pharm Degree/Provisional Certificate
    2. B.Pharm All Semester/Year Mark Sheets
    3. 10th Class Certificate/Mark Sheet
    4. 12th Class Certificate/Mark Sheet
    5. Government-issued Identity Proof
    6. Recent Passport-Size Photograph
    7. Signature
    8. Internship/Training Certificate, if applicable
    9. Transfer/Leaving Certificate, if applicable
    10. Migration Certificate, if applicable
    11. Registration Application Form
    12. Prescribed Registration Fee Receipt

    Important Note:

    For New D.Pharm / B.Pharm Pharmacist Registration, applicants are required to submit the necessary documents for verification through the email ID provided in the Contact section of the website.

    After successful verification, the applicant will be informed to deposit the prescribed ₹3,000 registration fee.

    Upon completion of all required formalities, the Pharmacist Registration Certificate will be issued as per the applicable rules.
